Certified Copy of Birth Certificate

Copy of parent driver’s license or photo ID

Copy of Last Report Card from previous school or transcripts for High School.

Immunization Record (available from child’s pediatrician) or waiver

Health Appraisal (front & back) completed by student’s physician (for kindergarten or to play on sports team) or waiver to signed.
